So this may be a glitch without victims, but it has occurred for me for a few months now. In iOS, on those (rare!) occasions that I give a wrong multiple choice answer, most of the time the system does not register the answer as being wrong (which I would expect to be a buzzer sound, and then would have to do the question over again eventually), but gives me a second chance and then registers the answer as being right. (Details: in Review, Vocabulary mode.)

So this problem clearly doesn’t strike a bell with others, but it still keeps happening.
