I’m a bit uncomfortable with this Chinese sentence. Sure, sometimes 是 means 有, but does it mean that here?. The sentence seems to mean “He primarily is a mental problem”. Maybe “His trouble was chiefly mental” would be better translated to 他的问题主要是心理上的。
