私は失敗の説明をした。

English Translation

I explained the failure.

The English translation was revised on Tatoeba side.

Old (= current Clozemaster version): I explained the failure.
New (= current Tatoeba version): I accounted for the failure.

The Japanese sentence doesn’t match the Tatoeba version in English…
私は失敗の説明をした means “I, as a third person, explained the background and root-causes of the failure (like a commentator usually does).” However, “I” am not responsible for the failure.

“I accounted for the failure” should be

私は失敗の釈明をした。(しゃくめい)or
私は失敗の弁明をした。(べんめい)
