“Avoir grand faim” is pretty rare these days, especially in spoken French.

“Avoir très faim” or “avoir vraiment faim”, are more commonly used to translate “to be very hungry”. So here for instance, you could say:

Tom et Marie ont très faim.
Tom et Marie ont vraiment faim.
