Tom headed toward the door.

Shouldn’t this be translated to “Tom started walking towards the door.”?

The current English text does not really imply “började”

Maybe not quite, but doesn’t the simple past tense in English sort of suggest it, compared to if the past continuous had been used?

But by all means, “Tom rörde sig mot dörren” is arguably a truer translation.
