I think this is incorrect translation? Graça seems to mean “grace”. So would this statement mean “What is your grace?” ?

Why was this translation changed? It is a very old fashioned way to ask for someones name. Here a source with the meaning what is your name: Qual a sua graça?

Not really. It means “What is your name?” though I’ve heard it only once in about 30 years and had to ask what it means.
