The italian sentence I would translate as custodian (false friend?), janitor, caretaker.
Keeper might be used in that sense but is unusual, isn’t it?
My first idea hearing the English one is (goal-)keeper, which is a “portiere”. 
Surely not a fish not to be thrown back because then it would be “It’s a keeper” (even if the sex is obvious to the fisherman).
Ciao di nuovo. I think tis best to think of this phrase as fairly up-to-date slang! 
A play on words. I’d be somewhat reluctant to decide on the sex of a fish b4 undoubtedly tossing it back into its watery home.
