I believe “combien” is better, because it expresses a degree. Using “comment” sounds to me like saying “I know in what way you are busy”.
Actually I think the “how” in “I know how busy you are” is not the same “how” as in “I know how it works” - they just happen to coincide. In French, they are different words.
