You’re not alone. I’ve never heard this expression (the English one) before. But I can kind of visualize how it might mean really has to go to the bathroom. (It’s not a pretty image!)
I’ve never heard the French one either.

Cheech said it in the movie “Up in Smoke“ in 1978 = “Get me outta here man! I gotta go! My back teeth are floating, man.” So I’ve heard it in the US, although I’ve never said it myself. I asked a friend of mine that knows these things. Neither one of us knows if we heard it before that or not. It does mean that there is a lot of urgency to pee. It isn’t vulgar but it isn’t used in polite conversation either. It might be a little outdated, I haven’t heard it for a long time.
