Ha un dente guasto.

English Translation

She has a decayed tooth.

I think guasto describes a broken tooth rather than one which is decayed.

Possibly un dente cariato is better? The verb for tooth decay seems to be cariarsi and WordReference suggests the sentence Mi si è cariato un molare.

I wonder, can one say then “Ho un dente cariato”?

Yes, I believe so. Cariato is an adjective as well as the past participle.
