Literally “can i follow you home” - is this how you would ask to walk someone home?

It should be “följa med”, unless they are going in separate cars or something like that.

One more thing: “att följa med någon hem”, implies that you are invited in when you get there. So I would suggest “Får jag följa med dig på vägen hem (på hemvägen)?”, to avoid misunderstandings.
