I truly do not understand the question.
I think the meaning of ‘oikeasti’ is milder than ‘truly’. Wiktionary suggests ‘really’ or ‘actually’.
“Truly” is normally used with statements of opinion, feeling, or emotional subtext, whereas “actually” is normally used with objective statements of fact. (“Really” can go either way.)
So, “I truly don’t understand the question” typically means something like “I can’t comprehend why you would ask that”, “I’m not sure what you’re trying to insinuate”, etc.
Here, there’s no way to tell whether this statement is intended to mean “I don’t understand the particulars of the question” or “I don’t get the motivation/insinuations behind the question”. So I agree that “really” is the best word choice, because it’s the only choice that’s flexible enough in its typical usage to convey either of these.
There’s also the matter of “I really don’t understand the question” (= I have absolutely no clue) vs. “I don’t really understand the question” (= my understanding is only partial).
“En oikeasti ymmärrä” is more likely to mean the weaker of these two statements, “I don’t rlly understand”. For “I rlly don’t understand […at all]”, most Finnish speakers would express the absoluteness of the confusion/lack of understanding with some sort of -kaan/-kään form—e.g., En ymmärrä kysymystä ollenkaan.
