Is this really a correct translation? Could a native speaker help?

“aus dem Konzept bringen” means “you’re confusing/distracting me”, “you’re making me lose my focus/concentration”. If the english “off balance” can have this figurative meaning, then yes.

Otherwise, Google Translate suggests “You’re throwing me off track”.

Interestingly, english wiktionary does not list this meaning of “Konzept”, and the german wiktionary has it listed under “Redewendungen”, but they are all red links.

“Off balance” can be used either literally or figuratively in English. Without more context, I would assume that someone was using it figuratively, i.e. it has the exact meaning that you’ve outlined here. So yes, it seems like a good translation.
