Co jest nie tak? (lit. What’s wrong?) or Co się dzieje? (lit. What’s going on?) would be more appropriate.
I don’t agree. Imagine a pupil asking a teacher or their colleagues what’s wrong on the test because the teacher highlighted their answer as wrong but they don’t understand why. Co jest źle? or Co jest tutaj źle? sounds the most natural for me.
